If you are planning to deliver your baby at Sutter Amador Hospital's (SAH) Family Birth Center, you have the option of completing online pre-registration.
Complete the Online Pre-registration Form today.
Alternately, you can complete the form provided by your obstetrician and mail it in.
Many insurance companies require pre-certification for hospital admissions. Please check with your insurance carrier.
